Generate Medicare Forms:

Get required CMS Scope of Appointments and Permission to Contact forms signed by leads & clients and stored within their files

What the Feature Does

The Generate Medicare Forms feature pulls verified client data from their record and auto‑populates required Medicare forms—cutting manual entry, reducing errors, and simplifying CMS compliance.

There is an option to use Medicare Forms in Automated Workflows and Appointment Scheduling. Just set it and forget it! And if your client forgets to sign their SOA, there are even reports for you to review the status to follow up with them.


SET UP

1. Enable Medicare Forms

Go to:
SETTINGS > Email, Columns, Calendar & Other
✔ Check Electronic Scope of Appointments and select the templates
Scroll down and Save

2. Agent Setup (Required)

Each agent must add their agent details:
Go to:
Account > My Information > SOA

Add:

  • Sales Rep ID #

  • Signature

Save your changes.

If this step is skipped, forms won’t generate correctly. Save yourself the trouble of having to go back and do it later.

3. Generate the Form

Go to the Client or Lead record and click Generate Medicare Form

Select:

  • The form type

  • The agent

  • Initial method of contact

Submit.

📧 The client receives an email with the form to sign.

4. Automatic Storage

Once signed and submitted:

  • The completed form is automatically saved

  • You’ll find it under the client’s Files tab

No manual uploads. No loose paperwork.

⏳ Expiration & Renewal

Both SOA and Consent forms must be renewed annually.

If the coverage year changes, your documentation needs to change too. One best practice is to implement a Scope of Appointment and Permission To Contact action at the beginning of your Enrollment Season workflow. You are required to retain consent documentation for a minimum of 10 years.
